The 2023 health history form has been mailed to all registered campers. Subsequent applicants will receive one following enrollment and acceptance. If at all possible, please use this mailed form, which a folded, single sheet of 11×17″ paper. If necessary, a four-page version of this form may be downloaded below and printed on 8.5×11″ paper.

The form should be signed by a physician and returned within four weeks of the start of a camper’s session (i.e., no earlier, if feasible). Failure to submit this form in advance will result in a delay in check-in.

Following the Centers for Disease Control and Prevention (CDC), Nature Camp recommends that all campers receive the tetravalent meningococcal conjugated vaccine (MCV4).

Please note that the back page of this form also serves as our means of collecting emergency contact information, which is required by Nature Camp’s crisis management policy.
